COVID-19 Screening Questions and Appointment Confirmation

Please review the information below and and make sure to confirm your appointment by replying “Confirm” to the text message you have likely received or call the office.  We are required by State of Michigan OSHA to screen our patients for COVID-19 before and during your appointment. THANK YOU for your patience and understanding as we strive to maintain a safe environment for everyone!

1. In the last 14 days have you had any symptoms of COVID 19?
(fever, cough, shortness of breath, sore throat, diarrhea, chills, muscle aches, headaches, or loss of
sense of taste or smell)

2. Have you been around anyone who has tested positive for COVID-19 in the last 2
weeks?

**If you have any COVID 19 symptoms or have been exposed to COVID 19 please call our office at 989-224-2319 regarding your appointment.** 

3. We are following the CDC and Michigan Dental Association guidelines for healthcare offices and require ALL employees, patients and visitors to wear a mask. 

4. When you arrive for your appointment please verbally check in at the front desk. You no longer have to call from the parking lot. 

If you are unable to make your appointment, it is important that you call us at 989-224-2319 as soon as possible so we can make other arrangements.
